At Academic HealthPlans (also referred to as “AHP” throughout this page and website), a Risk Strategies Company, we know how important your privacy is to you, and it is very important to us as well. Whether you are a current customer or just visiting the website to which this policy is linked (the “Website”), we are committed to attempting to protect the information you provide, just as we protect it when you provide it to us over the phone, in person or through the mail.

We value your relationship with us, so we want you to understand our privacy objectives for the Website, and how and why we collect, use and disclose the personal information about you on our websites. If you are a customer of AHP, you have also received the HIPAA Notice of Privacy Practices that describes AHP’s use and disclosure of your “protected health information” under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). This Website Privacy Statement supplements that HIPAA Notice by providing you with information concerning AHP’s practices and procedures relating specifically to information we collect at this website.

Acceptance of Privacy Policy

By using the Website, you signify your acceptance of this Privacy Policy.  If you do not agree to the terms of this Privacy Policy, please do not use the Website.  Your continued use of the Website following the posting of changes to this Privacy Policy will mean that you accept those changes.

Personal Information We Collect and Use at This Website

We do not collect any personal information about you at our Websites unless you choose to provide it to us voluntarily. We define “personally identifiable information” (“PII”) as information that identifies you as an individual and might include your phone number, name, age, address or Social Security Number. If you encounter a screen or page that requests information you do not want to share with us, do not enter the information and do not proceed with that screen or page.

We obtain information that you provide to us online – for example, when you submit an application or otherwise communicate with us. In some cases, we retain the information you provide to us online – for example, to complete a transaction you requested or to keep historical records of your past transactions.

You may have the opportunity to provide information to us through online surveys and forms. We use this information to better understand health care needs and to provide quality products, services and other opportunities. AHP wants your visit to our website to be informative and enjoyable. We plan to use any information we obtain from you to help us continually improve the content, functionality and overall usefulness of our website.

If you do provide us with PII, we will restrict its use to the purposes expressed where it was collected, except as described in this Privacy Statement.

AHP uses Plaid Technologies, Inc. (“Plaid”) to gather an insured’s data from financial institutions. By using our service, you grant AHP and Plaid the right, power, and authority to act on your behalf to access and transmit your personal and financial information from the relevant financial institution. You agree to your personal and financial information being transferred, stored, and processed by Plaid in accordance with the Plaid Privacy Policy.

Disclosure of Personal Information

AHP will not sell, rent, license, or trade your PII with third parties for their own direct marketing use unless we receive your express consent to do so.  Unless you give us permission to do so, we will not share your PII with third parties other than as specified in this Privacy Policy.

Disclosures to Third Parties Assisting In Our Operations.  We may share your PII under confidentiality agreements and any legally required data processing agreements with other companies that work with, or on behalf of, AHP to provide products and services, such as those who are providing email solutions, providing cloud hosting services, analyzing data and usage of the Website, or providing support and maintenance services for the Website, as well as legal, regulatory, audit and other professional advisors.  These companies may use your PII to assist us in our operations or for our legitimate business interests.  However, these companies do not have any independent right to share this information.

Disclosures Under Special Circumstances. We may provide information about you, including PII, to respond to subpoenas, court orders, legal processes or governmental regulations or inquiries, or to establish or exercise our legal rights or defend against legal claims.  We believe it is necessary to share information in order to investigate, prevent or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, or as otherwise required by law.

Other Parties With Your Consent or At Your Direction. In addition to the disclosures described in this Privacy Policy, we may share information about you with third parties when you consent to or request such sharing.

Business Transfers.  We may share your PII with other business entities in connection with the sale, assignment, merger or other transfer of all or a portion of AHP’s business to such business entity.  We will require any such successor business entity to honor the terms of this Privacy Policy.

Use of Email

For purposes of this site, email can be used either as a mechanism to provide information you requested or as a way we can communicate with each other.

In some situations, you may have provided your email address to us in order for us to send you information about a service or resource that you requested. Unless you opt out of receiving these types of emails, we will continue to send you information. In addition, we may use third-party vendors to determine if you open and use any information that we share via email, as well as to solicit your feedback through online surveys. Responding to these surveys is always optional.

Email can also be used as a mechanism for you to ask questions and provide comments about our Website or services. You are encouraged to provide this feedback. We do share these messages with individuals in the organization who can appropriately respond to the specific email. In addition, we may preserve the content of your email, your email address and our response.

It is important to use good judgment when sending information via the Internet to an email address. Email sent via the Internet may pass through private and public networks with varying levels of security. Some networks may have taken steps to secure email transmissions while others have not which could affect the privacy and integrity of the email.

Confidentiality and Security

We restrict access to personal information collected about you at our Websites to our employees, our affiliates’ employees, or others who need to know that information to provide services to you or are conducting our normal business operations. Your protected health information may be shared electronically with these parties or other authorized entities as permitted by state and federal law. We maintain appropriate physical, electronic, and procedural safeguards to protect your personal information.

Please note the advice above, under “Email,” regarding sending confidential, personal or sensitive information by email. If you do provide your personal information to us, such as an email, telephone or fax number, or other identifying information, we do not plan to disclose it without your consent. We will plan to maintain this information, as well as any other transactions, according to our usual security and confidentiality standards.

For your convenience we may provide links to other websites and web pages that we do not control. We are not responsible for the privacy practices of any websites or pages not under our control. We do not endorse any of these websites or pages, the services or products described or offered on such websites or pages, or any of the content contained on those websites or pages.

Use of Cookies or Other Data Collection Tools

Response to “Do Not Track” Signals

Some web browsers may transmit “do-not-track” signals to the websites with which the user communicates, although web browsers incorporate and activate this functionality in different ways, and it is not always clear whether users intend for these signals to be transmitted. There currently is disagreement, including among participants in the leading Internet standards-setting organization, concerning what, if anything, websites should do when they receive such signals.  AHP currently does not take action in response to these signals, but, if and when a standard is established and accepted, we may reassess how to respond to these signals.

Children’s Privacy Protection

Under Age 13.  AHP does not knowingly collect or retain personally identifiable information about persons under 13 years of age. Any person who provides their personal information to AHP via the Website represents that they are 13 years of age or older. The Website does not knowingly solicit or collect personally identifiable information online from children under the age of 13 without prior verifiable parental consent.  If we learn that a child under the age of 13 (or such legally required higher age) has submitted personally identifiable information online without parental consent, we will take all reasonable measures to delete such information from our databases and to not use such information for any purpose (except where necessary to protect the safety of the child or others as required or allowed by law). Under Age 18.  Minors under 18 years of age may have the PII that they have provided to us through the Website deleted by contacting us at [email protected] and requesting deletion.  Please note that, while we make reasonable efforts to comply with such requests, deletion of your personal information does not ensure complete and comprehensive removal of that data from all our systems and back-up systems.

Privacy Settings/Opt Out/Changes/Access

If you would like your PII removed from our database, changed or updated, you can contact us at [email protected].  If requested, we will promptly verify and delete your account and you will no longer receive emails from AHP. Your removal from the mailing list or database will not remove records of past transactions or delete information stored in our data backups and archives where we are required to keep your data for legitimate business or legal requirements. Data on past transactions and data stored in backups and archives will be deleted in the normal course of our business.  You have the right to access or, if required by applicable laws, receive a copy of your PII held by us by making a written request by sending an email to the contact email address below.

Website Is Directed Solely to United States

This Website and AHP’s services are only intended for, and directed to, customers and users located in the United States.  AHP and its servers are located in the United States and are subject to the applicable laws of the United States (where data privacy laws are less stringent than in the European Union and some other jurisdictions).  If you choose to visit the Website or use AHP’s services, you consent to the transfer, use and disclosure of information in accordance with this Privacy Statement and subject to such applicable laws.

Changes to our Website Privacy Statement

This Privacy Statement may be revised from time to time as we add new features and services, as laws change, and as industry privacy and security best practices evolve. If we update the Privacy Statement, we will let you know about our changes we consider material by placing a note on the relevant web page you use to access the Services or you may receive an email with notification of the updated Privacy Statement. By accessing or using the Website after we have placed such a notice, you consent to the new practice(s) identified in the update.
